This should be filled out by the Digital Navigator after they’ve received a device and taken recommended classes and/or after they have finished ILPs and one-on-one assistance and you haven’t heard from them for more assistance for at least three months.

Is this participant part of a device program?
To start with, do you remember what your goals were when you first contacted NDEC? For instance, did you need a device and if so what were some of the things you wanted to do with a device?
If no or not, sure please remind them of some of the reasons why they wanted this device.
Understanding that there may be more to learn and you can always take more free digital skills classes at NDEC, how much progress do you feel you made towards those goals?
Were NDEC Digital Navigator services and classes delivered in a way that made you feel comfortable and respected? For example, were services delivered in a language you understand and were your accessibility needs met?
If no, please let them know someone from the management team will be contacting them for further feedback.
We'd like to share your story in your own words as much as possible with others, but we want to do it only in a way that works for you.
If they agree to share their name with their story, please ask if they would be willing to meet with you and sign a photo release form. Please let them know that you may be taking a picture or we may ask someone to assist us with pictures and/or videos.
Are any of these things that you would like to learn? It is okay to continue learning with NDEC and we will develop a plan to help you with these skills either through additional classes or one-on-one assistance.
Especially if they say that they are not confident with keeping their personal and financial information safe, please say: would you like to hear about the three other internet safety classes that we have? I can help you get enrolled in those classes and if there isn't one coming up that you can make, I can let you know when I have arranged one.
Please go over Keeping Your Online Accounts Safe, Cloud-based BackUp Solutions, and Using Facebook Securely with them and help them enroll in any that they request.
If there are no more digital skills goals and classes that they need, please ask: Do you know how to find and register for classes on your own and do you have my contact information if you are not seeing a class that you would like to take in your area?
If they don’t, spend time giving them this information.
If this is the final session, meaning they don’t wish to extend the ILP beyond required device classes, please thank them for their participation and encourage them to keep learning!
